1. PARTIES:

The terms of this agreement are made by and between _________________________ (the commissioner) and Galina Perova (the artist).

2. NATURE AND EXTENT OF SERVICES:

The service to be performed by Galina Perova will be the creation of an oil on canvas portrait of _______________________________ . Said portrait will be approximately _____ x _____ inches. The painting will be completed no later than _______________. This deadline may be renegotiated with the approval of both parties.

3. COMPENSATION:

The commission fee is $ ____________________. Utah residents are also obligated to a 6.35% Utah State Sales Tax. A 50% deposit is due at the commencement of the project. The unpaid balance is due upon delivery. The deposit is non-refundable except in the event of illness or death of the Artist, or if for any reson the portrait is not completed.

4. CONDITIONS:

In the event that the Commissioner is not satisfied with the portrait, the total obligation of the Commissioner is limited to the original 50% deposit. In addition, there will be no obligation to display the painting.
